This does not demonstrate a reason to taking over health care. It only demonstrates a need to beware, educate the public, and insure doctors can’t monopolize their local publics. Gov’t can’t use the excuse that abuse may happen in order to take over everything!!! Gov’t’s # 1 responsibility is to protect us from enemies, foreign and domestic, not protect us from our own enemy within – our stupidity. Oh I can not take this crap any longer he does not know what the heck he is talking about..here is an example of that …my first husband was a diabetic he weighed 134 pounds soaking wet…he had a sore on his foot that would not heal went to the VA hosp ended up the longest hospital stay of all time first they took part of the foot off then after 6 months open wound not healing he ended up with gangrene…at the point of deaths door I took a lawyer to the hospital with me to speak with the doctors,,the lawyer asked if my husband would require more surgery they said no.. well the very next day they told him they will have to amputate his leg…That my friends is your government health care!

The only way to “save” money by raising payment for preventative is if doctor’s are not doing preventative so that they could get the $50,000 for the amuptation.

1) Doctor’s are not taking care of people correctly so that they can get a pay out later for the major problem it causes
2) The doctor and the surgeon are in on it together and the surgeon who amputates is kicking money back to the doctor who didn’t treat the diabetes right on purpose.

This man is a moron. He knows very little of which he speaks. My wife is a 52 year old, controlled diabetic who is not overweight, eats right, and works out on a regular basis. She see’s an Endocrenologist 4 times a year with a $40 copay, the Dr. is reimbursed an additional $168 from our insurance carrier. That’s $832/yr for “preventive” medicine. My uncle Sam, who is a 74 year old diabetic recently had his foot amputated due to this disease. Medicare reimbursed the surgeon $419 for the surgery.
I don’t see much incentive for the surgery!

If people end up with complications of diabetes, it isn’t the health care parctitioner’s fault, it is the PATIENTS fault for not being compliant! To insinuate that if a diabetic doesn’t have their disease under control it is not only the fault of the doctor, but the preferred outcome for the doctor so he/she can earn more money is so offensive. Honestly – I am trying to understand this President – but have to say – Barack Obama is completely clueless. Most physcians treat the effect and not the cause. It’s impossible to monitor each patients lifestyle. Everyone would have to have their own personal phys. like the pres. Montoring is code for controlling what people eat, what pills they take, how they sleep etc. its impossible to monitor people when a doctor sees one person for 5 minutes.
Preventive medicine has been around in the form of HMOs’ for decades.This form of coverage dole out care and you are limited to what doctors you can see and what procedures they can perform. Its like getting so many credits and when you use them up your @hit out of luck.
This is to large an issue to ram thru in 6 months or even a year. Its to big an issue to hand over to the government exclusively.

The only way to pay more preventative and have it save money overall is to have doctors choose to do preventative instead amputation. Meaning they aren’t doing preventative now because they get mo0re for amputating.

I only do amputations when no other alternative exists such as lesser operation, medical treatment or revascularization of the extremity. To suggest that I would permanently maim someone for financial gain hurts me to my core. And the reimbursement I get is in line of $500-900.00 not $50,000.00. Take my 55% overhead and my 50 % tax rate and I take home $125.00 to $225.00. The entire cost to the system including hospital fees, other physicians, artificial limb etc may well be $50,000.00 or more but I don’t see that. Folks you have just be lied to. And you wonder why most doctors are against these idiots running anything.

Hmmm… did I miss any brilliant moves by our “Genius in Chief”?

Unbelievable lack of insight into this issue from the guy who’s supposed to be making the difficult decisions.

First of all… it is MUCH less expensive to amputate an extremity in a patient with a an infected diabetic wound than to make heroic attempts at salvaging the leg – and most of the time end up with an amputation in any case. If you want to argue economy you would be better off immediately amputating a patient’s leg once it gets to that end point.

Secondly… surgeons reimbursed $40,000 for an amputation? Is he crazy? Medicare pays less than $1000 for an above-knee amputation and given the amount of time spent with the patient/family before an after surgery, the high-risk nature of the surgery, this is a surgery that’s hardly worth doing from a monetary standpoint.

Where is this guy getting his facts? Does he feel like it’s OK just to spout whatever he wants? This is getting disgraceful.

Truly this is what we need to “tweek” healthcare. We need “medical” professionals…not “professional” politicians who know nothing of healthcare.

Experts like yourself will give a true assessment of where we are in healthcare and what needs to be done to correct it.

My suggestion is that it is examine piece by piece not a complete “overhaul” of our existing system. We do not need to throw the baby out with the bath water…we just need to change the dirty water!

We can start with prescription drugs. Get a panel of pharmacists, doctors, nurses, and drug suppliers along with “some” token politicians.

Not just from the AMA…in bed with Bilderberg Bush-Obama. But professionals who will volunteer their services to help correct the system.

Then once the prescription drugs are finished and passed…we can go on to Emergency rooms and their problems and systematically go through our “existing” healthcare and improve it for all!

As you read, the one thing that will repulse you most is when you read the section that describes dismemberment and loss of limbs.

Mr. Obama chooses the MOST FRIGHTENING and disgusting language from insurance policies and uses it to score political points.

Given the cost of treating an ulceration on the extremity, and Obama’s obsession with saving money, what he should do is just tell diabetics that if they don’t take care of themselves and end up with an incurable sore, they will not be given any treatment to save the foot, we’ll just have it amputated because that will be cheaper than trying to save it!

What a complete and utter MORON! I am a surgeon that does amputations on diabetics. I can assure you that I have NEVER been paid 40 to 50 THOUSAND dollars for cutting off a leg. In fact the average reimbursement for a BKA or AKA is around 500 to 600 bucks.

BTW, if the family care providers want to be paid more then go thru the extra 3 to 5 years of training, and go into a specialty that pays. Family docs will be extincet because what they do can be done by PAs and NPs bottom line. They are a non-specialty.

As an orthopedic surgeon this is laughable that a surgeon makes $30,000 to $50,000 for the amputation “of a foot”. The truth is $300 to $600 dollars, and that includes the pre-op visit and three months follow-up in the office. To suggest we would amputate a limb unnecessarily (or for profit) is unconscionable. Nothing could be further from the truth. We should be scared to think this is the brain trust of health care reform.

First,I have yet to hear of surgeon getting reimbursed anywhere near the dollar figure he throws out, it is typically less the $1,000 to the surgeon. Yes, that is still a lot of money but surgeons also have years of training and more responsibility than most of us in our jobs. Obama is misinforming Americans and this is wrong!

Second, most family practitioners are working hard to prevent but many Americans want an easy fix and do not take care of themselves as instructed by their PCP. So they end in the ER with a massive infection and needing an amputation, oh, and may be one of the many patients that a surgeons takes care of without billing at all.

Third, Americans need to be accountable and Obama needs to get his facts straight!

This kind of ignorance makes me crazy. Its bad enough to deal with the disinformation
from the Right;we’ve got it coming from the Left as well.
Does Obama have a proof-reader for his speeches? Any Certified Coder could have told him
that the Surgeon receives $586.86 for the amputation of a foot (CPT Codes 2007-Medicare
reimbursements).
Combine that with the three month “global period” of free postoperative care for that
particular surgery. Meaning …that every postoperative visit,(hospital and office) is
for free.
So if the patient is in the hospital for a week, (he probably has other co-morbidies
such as diabetes and cardiac disease). The Surgeon sees him daily, plus at least two
or three visits to the office.
He gets about $58.00 per visit to share with his office staff and pay for supplies.
You pay a plumber more than that.

I am a Head and Neck surgeon and Mr. Obama is way off base. There are no surgical
procedures that reimburse anywhere close to $30,000. Even a complex 10 hour proc
edure will be at best $7,000, if Medicare more like $1,000. A forefoot amputation
likely nets the surgeon around $500. Our president is dangerously inaccurate and
misleading and needs to be held responsible. This is an outrage.

Where is the AMA; are they not going to make public statements contradicting these outrageous comments from “The Smoker”!

I think it’s amazing how you can hear his actual words, yet somehow believe he said something completely different than what he said. He didn’t say that doctors choose amputation. He said that we should pay more money to primary care physicians to fight diabetes in order to prevent amputation, which costs more. That’s quite obvious from his words. Yet you people are so intent on finding something wrong that you refuse to believe your own ears.
